Alexis Life LLC is not a medical practice.
Healthcare services accessible through the Platform are provided by independent licensed healthcare professionals (“Providers”) practicing through independent professional medical entities (“Medical Practices”).
Alexis works with OpenLoop Health, Inc. to facilitate access to Providers and related clinical services.
This Privacy Policy governs personal information that Alexis collects or processes in connection with the Platform and Alexis’s non-clinical services.
Certain information relating to your health may also be subject to additional privacy protections.

Protected health information (“PHI”) created, received, maintained, or transmitted by a Provider or Medical Practice may be governed by the applicable Medical Practice’s Notice of Privacy Practices under the Health Insurance Portability and Accountability Act of 1996 (“HIPAA”) and other healthcare privacy laws.
When Alexis processes information on behalf of a Medical Practice or Provider rather than for Alexis’s own purposes, that processing may be governed by the applicable clinical privacy documents and contractual obligations rather than solely by this Privacy Policy.
2. Scope of This Privacy Policy
This Privacy Policy applies to personal information Alexis collects:
- on or through the Platform;
- when you create or manage an Alexis account;
- when you communicate with Alexis by email, telephone, text message, chat, or other electronic means;
- when you purchase, enroll in, or request Alexis products or services;
- when you interact with Alexis advertising, communications, or services on third-party websites or platforms where this Privacy Policy is linked or referenced; and
- from service providers and business partners where permitted by law.
This Privacy Policy does not govern:
- PHI maintained by a Provider or Medical Practice and governed by its Notice of Privacy Practices;
- the independent privacy practices of a Pharmacy, laboratory, financing provider, healthcare professional, or other third party;
- websites or applications operated by third parties that have their own privacy policies; or
- employment-related information concerning Alexis employees or job applicants.
3. Personal Information We Collect
The information we collect depends upon how you interact with Alexis and the Services you request.
Contact Information
We may collect:
- first and last name;
- email address;
- telephone number;
- mailing or shipping address; and
- communication preferences.
Account Information
We may collect:
- account identifiers;
- username or login information;
- account credentials or authentication information;
- account preferences;
- Services or products associated with your account; and
- account activity.
Transaction and Order Information

- products or programs selected;
- purchase history;
- subscription or program information;

- transaction dates;

Payment Information
Payments may be processed by third-party payment processors.
Where payment-card information is collected directly by a third-party payment processor, Alexis generally does not receive or retain your complete payment-card number.

Eligibility and Treatment-Related Information
When you seek healthcare services through the Platform, you may provide information relating to your eligibility, treatment interests, health goals, or other health-related matters.
Health-related information may be subject to our separate Consumer Health Data Privacy Notice and, when maintained by a Provider or Medical Practice, an applicable Notice of Privacy Practices.
Communications and Customer Support
We may collect information contained in:
- emails;
- text messages;
- chats;
- customer-support requests;
- telephone communications;
- surveys;
- feedback;
- complaints;
- reviews; and
- other communications with Alexis.
Where permitted by law, telephone or support communications may be recorded or monitored for quality, training, security, or dispute-resolution purposes.
Information You Choose to Make Public
If you voluntarily submit a review, testimonial, comment, image, or other content for public use, information you authorize us to publish may become publicly available.
We will obtain any additional authorization required by applicable law before publicly using identifiable health information or patient testimonials where such authorization is legally required.

When you visit or interact with the Platform, Alexis and its service providers may automatically collect information concerning your device, browser, and use of the Platform.

Alexis does not intentionally use identifiable clinical-intake information, prescription information, diagnosis information, or similar Consumer Health Data for cross-context behavioral advertising.
You may be able to manage certain cookies through browser settings, device settings, or privacy controls made available on the Platform.
Disabling certain technologies may cause portions of the Platform to function differently or become unavailable.
Where applicable law requires Alexis to recognize an opt-out preference signal, such as a qualifying Global Privacy Control signal, Alexis will process that signal as required by applicable law.
6. Information We Receive From Other Sources
Alexis may receive personal information from third parties where necessary to provide Services or conduct our business.
These sources may include:
- Providers and Medical Practices;
- OpenLoop Health, Inc.;
- Pharmacies;
- laboratories;
- payment processors;
- financing providers;
- identity-verification providers;
- communications providers;
- shipping and fulfillment providers;
- fraud-prevention and security providers;
- analytics providers;
- advertising or marketing providers; and
- other service providers or business partners.
The information received depends on the relationship and the Service involved.
For example, Alexis may receive information confirming that a Provider has completed clinical review, that a prescription has entered fulfillment, or that an order has shipped.
7. How We Use Personal Information
Alexis may use personal information for the following purposes.
Providing and Operating the Platform
We may use information to:
- provide the Platform;
- create and administer accounts;
- authenticate users;
- process transactions;
- administer purchases, subscriptions, cancellations, and refunds;
- facilitate Services you request;
- provide order and fulfillment information; and
- provide customer support.
Facilitating Healthcare Services
Where you request healthcare services, Alexis may use or transmit information as necessary to:
- connect you with Providers and Medical Practices;
- facilitate clinical intake;
- coordinate healthcare Services;
- support communications;
- facilitate prescription fulfillment after a Provider issues a prescription;
- coordinate with Pharmacies or laboratories; and
- otherwise administer the non-clinical aspects of Services you request.
Providers, not Alexis, make clinical decisions.
Communications
We may use information to:
- send account notices;
- respond to inquiries;
- provide transaction or order information;
- provide appointment or fulfillment information;
- send security notifications;
- communicate changes to Services or policies; and
- provide other transactional or service-related communications.
Marketing and Advertising
Subject to applicable law and your communication preferences, Alexis may use appropriate personal information to:
- send information about Alexis products or Services;
- provide newsletters or promotional communications;
- measure the effectiveness of advertising;
- understand audiences; and
- improve marketing campaigns.

Platform Improvement and Analytics

Providers and Medical Practices
When you request healthcare Services, information may be disclosed to the appropriate Provider or Medical Practice to facilitate the Services you requested.
A Medical Practice’s handling of PHI may be governed separately by its Notice of Privacy Practices.
Pharmacies and Laboratories
When applicable, information may be disclosed to independent Pharmacies, laboratories, or other healthcare service providers as necessary to provide requested Services.
Service Providers
Alexis may disclose information to companies that perform functions for us, including:
- website hosting;
- cloud infrastructure;
- payment processing;
- communications;
- customer support;
- security;
- identity verification;
- fraud prevention;
- analytics;
- advertising and marketing;
- shipping coordination;
- record management; and
- professional services.
These providers may access information only as appropriate for the services they perform and subject to applicable legal and contractual requirements.
Payment and Financing Providers
Information may be disclosed to payment processors or financing providers when necessary to process a transaction or financing arrangement you request.

- other Service-related messages.
If you separately consent to promotional text messages, message frequency may vary and message and data rates may apply.
Consent to receive marketing text messages is not a condition of purchasing goods or Services.
Where supported, you may opt out of promotional text messages by replying STOP and request assistance by replying HELP.
Alexis does not sell mobile telephone numbers or SMS opt-in information.
Alexis does not disclose SMS opt-in consent to third parties for their own marketing or promotional purposes.
We may share mobile information with communications vendors, carriers, and service providers as necessary to operate the messaging program.

Marketing Emails
You may unsubscribe from promotional emails by using the unsubscribe link provided in the email.
You may continue to receive non-promotional communications concerning your account, transactions, security, healthcare coordination, or Services.
Text Messages
You may opt out of promotional SMS messages by replying STOP where supported.
Cookies and Similar Technologies
You may be able to restrict certain cookies or similar technologies through:
- browser settings;
- device settings;
- Platform privacy controls, where available; or
- other legally recognized privacy mechanisms.
Some Platform functions may not work properly if certain technologies are disabled.

Depending on your state of residence and whether applicable privacy law applies to Alexis, you may have the right to opt out of certain targeted advertising, sales, sharing, or profiling activities.
Where applicable law requires recognition of browser-based opt-out preference signals, Alexis will process qualifying signals as required by that law.

12. De-Identified and Aggregated Information
Alexis may create or receive information that has been aggregated or de-identified so that it cannot reasonably be linked to an identified or identifiable individual.
We may use and disclose properly aggregated or de-identified information for lawful purposes.
Where required by applicable law, Alexis will maintain de-identified information in de-identified form and will not attempt to re-identify it except as permitted by law.

Alexis retains personal information for as long as reasonably necessary for the purposes for which it was collected, including to:
- provide Services;

- complete transactions;
- provide customer support;
- comply with legal, tax, accounting, contractual, or regulatory obligations;

Retention periods vary depending upon the type of information, the purpose for which it is processed, legal requirements, and applicable contractual obligations.
Healthcare records independently maintained by Providers or Medical Practices may be subject to separate medical-record retention requirements.
When personal information is no longer reasonably necessary, Alexis may delete, anonymize, or de-identify it in accordance with applicable law and operational requirements.
14. Data Security
Alexis maintains administrative, technical, and physical safeguards designed to protect personal information from unauthorized access, acquisition, disclosure, alteration, destruction, or loss.
These safeguards may include, as appropriate:
- access controls;
- authentication;
- encryption;
- security monitoring;
- vendor-management practices;
- contractual protections;
- data-minimization practices; and
- other reasonable security controls.
No Internet transmission, website, network, or information system can be guaranteed to be completely secure.
You are responsible for maintaining the confidentiality of your login credentials and should promptly contact Alexis if you believe your account or personal information has been compromised.
15. Children
The Platform and healthcare Services are intended for adults.
Alexis does not knowingly offer healthcare Services through the Platform to individuals under eighteen (18) years of age.
If we learn that we have collected personal information directly from a minor in circumstances prohibited by applicable law, we will take appropriate steps to delete or otherwise address the information.
16. Third-Party Websites and Services
The Platform may contain links to or integrations with third-party websites or services.
These may include websites or services operated by:
- Providers;
- Medical Practices;
- Pharmacies;
- laboratories;
- payment processors;
- financing providers;
- shipping providers; and
- other third parties.
This Privacy Policy does not govern the independent privacy practices of third parties.
